Medical Cannabis Evaluations $149

Visit with Tracy Boller, APRN, Medical Marijuana Practitioner, to review your medical history to confirm that you have one of the conditions that qualify you for certification. 

The first medical cannabis evaluation lasts approximately 20-30 minutes and must be in person. Annual renewal visits may be done via telehealth. 

Authorization will be entered into the Kentucky Medical Cannabis State Registry for the patient. 

You must be a resident of Kentucky and either at least 18 years of age or have an assigned caregiver; have not been convicted of a disqualifying felony offense, and have medical records indicating that you have been diagnosed with a qualifying condition. If a condition needs to be diagnosed by our provider, additional costs will apply.

 Qualifying conditions for Medical Cannabis Certification:

• ​Cancer or terminal illness
  
• Chronic or severe pain
  
• Epilepsy or other intractable seizure disorder 
  
• Multiple sclerosis, muscle spasms, or spasticity
  
• Chronic nausea or cyclical vomiting syndrome
  
• Post-traumatic stress disorder (PTSD)
