Medical Cannabis Evaluations

During the evaluation, our provider will review your medical history to confirm that you have one of the conditions which qualify you for certification. 

Initial visits last approximately 20-30 minutes and must be an in-person visit.

You must be a resident of Kentucky and either at least 18 years of age or have an assigned caregiver; not been convicted of a disqualifying felony offense, and have medical records indicating that you have been diagnosed with a qualifying condition. If a condition needs to be diagnosed by our provider, additional costs will apply.

• ​Cancer or terminal illness
  
• Chronic or severe pain
  
• Epilepsy or other intractable seizure disorder 
  
• Multiple sclerosis, muscle spasms, or spasticity
  
• Chronic nausea or cyclical vomiting syndrome
  
• Post-traumatic stress disorder (PTSD)
