Initial Consultation-$149 

Your initial visit includes a comprehensive medical evaluation, screening for weight-loss treatment eligibility, and baseline lab work when indicated.

Insurance Notice:
Jomon Wellness is a self-pay / cash-pay practice. We do not bill insurance for services. This allows for longer visits, individualized care, and transparent pricing. 

Monthly Medical Weight Management Programs

Programs start at $200 per month for injectable-based treatment plans and may include:

• Ongoing medical management and provider oversight
• Prescribed medications and required supplies
• Nutritional guidance and lifestyle education
• Follow-up visits, ongoing support, and regular check-ins

Payments are set up as convenient recurring monthly billing. There is no long-term contract, and you may discontinue at any time. 

For patients who prefer to prepay, discounted 6-month and 12-month packages are available when paid in full.

Individualized treatment plans may include:

• Oral medications
• Injectable therapies                                                    
• Oral and injectable supplements and vitamins
• Compounded formulations when clinically appropriate

All options are reviewed and determined by your provider based on medical history and treatment goals.

Telehealth & Laboratory Options

Telehealth visits are available for eligible patients. When labs are required:

• Lab orders can be sent directly to you, allowing you to have labs drawn at the location of your choice
• You may choose to have labs billed to your insurance through an outside lab, if applicable (Jomon Wellness does not guarantee pricing of labs drawn and billed through an outside lab)

For patients who prefer cash-pay labs, we offer in-house lab coordination at a flat rate of $50 per lab set.

Follow-up labs are typically repeated every 3–6 months, depending on your individualized treatment plan.

Additional Policies

Ongoing education, support, and follow-up are included in program pricing.
Repeat labs ordered as part of your individualized treatment plan are billed separately.

A $50 fee will be charged for no-shows or cancellations made with less than 24 hours’ notice.
